Black Hills Regional Eye Institute in Rapid City reports a January cyberattack exposed patient data.
The institute says an unauthorized party accessed files between Jan. 4 and Jan. 8. A review finished July 30, and notification letters began Aug. 29.
Information may include medical and insurance details and, in some cases, Social Security numbers.
Black Hills Regional Eye Institute says there is no evidence of misuse and is offering credit monitoring.
